Dynamic image fields in templates and interview

Allow a user to upload images during the interview that will be inserted into the assembled document and allow image placeholders (INSERT IMAGE) to specify exact image size and positioning within the document

We have an Epic in our backlog for this item. This is a fairly large feature that needs to cover if and how images are stored in relation to the answer file.
